INVITATION TO PARTICIPATE IN THE 2011 FAMILY PACK PROMOTION

Your service may have participated in the 2010 Family Pack and Australian Family Magazine Promotion. You are now invited to partake in the 2011 promotion.

Please complete the following form accordingly.

By submitting this form your service agrees to the following:

1. Only distribute the Family Packs and Australian Family Magazine to parents and or guardians.

2. Not sell the packs in any way.

3. Not place any other items in the Family Pack and Australian Family Magazine.

4. Parents and Guardians are under no obligation to take the packs.

PLEASE NOTE: There is a limit of 6,000 Family Packs and magazines to be distributed. Allocation of these packs will work on a "first in" basis.

This must be a street address. We do not deliver to PO Boxes.
For confirmation purposes
The total number of Australian Family magazines and Family Packs delivered will depend on availability and will be provided at the discretion of Australian Family.
For internal use only.
AGREEMENT

This Service agrees to participate in accordance with the terms and conditions of the Family Pack and Australian Family Magazine Promotion 2011 and by doing so acknowledges responsibility for the distribution of Family Packs delivered to the Service.

From time to time Australian Family is approached to distribute samples to Services outside of the times that the normal "Family Pack and Australian Family Magazine" is distributed. If you would like to partake in the special bonus offers please indicate your interest below.
